Advance Directive

   An Advance Directive is a non-legally binding document that focuses on choices for the end of life. It is easily changeable and specifically applies to situations in which health recovery is unlikely. Within this document, a Health Care Proxy can be designated, legally empowering someone to advocate for your preferences.

   Filling out the Advance Directive is a valuable step in clarifying your wishes. This document, when completed,  presents a structured guide to both family members and medical providers. It serves as a vital tool for ensuring that your healthcare decisions align with your values even at a time when communications from you may be difficult or impossible. 

   Providing a clear and compassionate space, our Doula can work together with you to sort your feelings and values as you make your choices. We can also provide just the Advance Directive document for you to complete on your own.
